Adoption of the Plan
The Wexford Community Credit Union Health and Welfare Benefit Plan, effective 01/01/2013, as
amended and restated herein, is hereby adopted as of 09/01/2013. This document constitutes
the basis for administration of the Plan.
IN WITNESS WHEREOF, the parties have caused this document to be executed on this 6th day
