           Newborns’ & Mothers’ Health Protection

           Group health plans may not, under federal law, restrict benefits for any hospital length of stay in connection
           with childbirth for the mother or newborn child to less than forty-eight (48) hours following a vaginal delivery,
           or less than ninety-six (96) hours following a cesarean section.  Federal law does not prohibit the mother’s
           or  newborn’s  attending provider,  after  consulting  with  the  mother, from  discharging the  mother  or  her
           newborn earlier than 48 hours (or 96 hours as applicable), if the patient consents.



                                   Women’s Health & Cancer Rights Act (WHCRA)

                                   Did  you  know  that  our  medical  plan,  as  required  by  the Women’s  Health  and
                                   Cancer Rights Act of 1998 provides benefits for mastectomy-related services?
                                   Coverage will be provided, as determined in consultation with your physician, for
                                   the following procedures:

                                   •    Reconstructive surgery for the affected breast

                                   •    Surgery on the other breast to achieve symmetry

                                   •    Prostheses

                                   •    Treatment for complications from the mastectomy, including lymphedema

           Benefits for these services and supplies are subject to the same deductible and coinsurance limitations as
           for other services, supplies, and procedures that apply under the Kemba Charleston FCU Highmark West

           Protecting Your Privacy

           The Health Insurance Portability and Accountability Act of 1996 (HIPAA) requires employer health plans
           to maintain the privacy of your health information.  HIPAA also requires employers to provide employees
           with a notice of the health plan’s legal duties and privacy practices with respect to your health information.

           Qualified Medical Child Support Order

           These plans will provide benefits as required by any qualified medical child support order (QMCSO),
           as  defined  in  ERISA  § 609(a). The  Plan  has  detailed  procedures for  determining  whether  an  order
           qualifies  as  a  QMCSO.  Participants  and  beneficiaries  can  obtain,  without  charge,  a  copy  of  such
           procedures from the Plan Administrator.
