Long Term Disability (LTD) Coverage

Long Term Disability (LTD) provides income to employees who are
disabled for an extended period of time. The LTD plan from Sun
Life is designed to help protect your income in the event that you
are unable to work due to a covered disability.

Your coverage effective date or any increase in coverage may be
delayed if you are disabled on the date coverage is scheduled to
take effect. Review the Summary Plan Description for details.

LTD Benefits

Monthly Benefit 60% of Pre-Disability earnings

Elimination Period 180 Days
Minimum/Maximum $50 / $6,000 per month
Monthly Benefit
The later of Your Normal Retirement Age as defined by Social Security or the
period shown below:

Benefit Period 62 42 months
63 36 months
64 30 months
65 24 months
66 21 months
67 18 months
68 15 months
69 and over 12 months

LTD Benefits
3 Year Own/Any Occupation
For the first 36 months of disability, the employee is considered totally
disabled if he/she is unable to perform the material duties of his/her regular

occupation. After 36 months, the employee is considered disabled if he/she
is unable to perform the material duties of any gainful occupation for which
he/she is reasonably fitted by training, education or experience.

Pre-existing Conditions Claims filed in the first 12 months if not treated 3 months prior to effective date, or
had a 6 month treatment free period, is not pre-existing.
