Benefit Information:

 New hire benefits will be effective the first of the month

following 60 days of employment.

 Annual open enrollment is in October, and effective
November first.

 Any other benefit changes are allowed due to a life changing
event (new baby, adoption, loss of employment, etc.). Life

changing event benefit adjustments must be completed
within 30 days of the event.

 Benefits can be purchased separately (a la carte), you do not
have to bundle benefits.
