 Who is Eligible and When
Third Place – Elizabeth Scott
Eligibility for medical coverage is effective the first day of the month following 60 days of employment, status change or if rehired after an absence greater than 13 weeks. Employees have 30 days from date of hire, status change or rehire to elect medical coverage.
Regular full-time and part-time employees working 30 hours or more per week are eligible to participate in the Medical Plan with Agency contribution. Regular part-time employees working 20–29 hours per week are eligible to participate in the Medical Plan by paying 100% of the premium rate.
Spousal Medical Benefits Policy
Employers are constantly struggling with ways to control health care cost while continuing to offer a competitive and affordable benefit package for employees. Northern Human Services is no exception to this. Health care cost continue to increase at alarming rates and employers must use innovative methods to control the increases.
Northern Human Services will allow medical coverage for spouses whose employer does not offer large group medical insurance (over 50 employees).
If your spouse is not currently eligible for their employer sponsored plan, we will need a letter from their employer indicating ineligibility for benefits in order to continue their coverage on the Northern plan. All covered spouses will need to fill out a Spousal Affidavit form. Please direct questions to your local Office Manager or Human Resources.
Benefits You Receive
In 2020-2021, Northern Human Services will continue to offer comprehensive Medical Benefits to all eligible employees, unchanged from last year. We will remain with the Cigna Open Access Plus plan which offers national in-network coverage with no referrals needed. Your primary care and specialist’s visit copays will be $25. Outpatient labs and x-rays are covered in full. Please note that Emergency Room visits go to your deductible.
For the 2020-2021 plan year, your deductible resets (or begins anew) on July 1st and is $5,000 per individual or $10,000 per family. There is no carryover of your deductible from the last plan year. There is a Health Reimbursement Account (HRA) to offset some of the deductible expenses. Please see details outlined in the chart on the next two pages.
