 We consider you a partner in your hospital care. When you are well-informed, participate in treatment decisions, and communicate openly with your doctor and other health professionals, you help make your care as effective as possible. This hospital encourages respect for personal preferences and values of each individual.
WHILE YOU ARE A PATIENT IN THIS HOSPITAL, YOUR RIGHTS AND RESPONSIBILITIES INCLUDE:
YOU HAVE THE RIGHT TO...
• care that is considerate and respectful of your personal values and beliefs.
• receive the name of the person in charge of your care.
• have a family member and physician informed of admission.
• an environment safe from abuse, neglect, or exploitation.
• receive information necessary to give informed consent prior to the start of any procedure and/or treatment from your physician.
• be informed about and participate in the decisions regarding your care.
• your Medical Record. Information can be made available to a person on your behalf.
• expect that all communications and records pertaining to your care will be treated as confidential.
• participate in ethical questions that arise in the course of your care, including issues of conflict resolution and life sustaining treatment.
• make advanced care decisions in case you are unable to communicate your wishes.
• every consideration of your personal privacy and security while

• obtain information as to any relationships of the hospital to other health care or educational institutions. In so far as your care is concerned.

COMPLAINT RESOLUTION
If you have a complaint, we would like to have the opportunity to resolve it. Our goal is to respond to your complaint in a timely manner with an appropriate and clear explanation. It is the responsibility of all staff members of BMH to prohibit neglect, mental and/or physical abuse, including involuntary seclusion and misappropriation of property of patients/residents from staff, family, visitors or other patients/residents. The Facility’s leader-ship will conduct an internal investigation of victims of alleged or suspected abuse, neglect or misappropriation of property and will provide notification and the release of information to the proper authorities. If you suspect any incident of abuse or neglect, you have the responsibility to report it to your supervisor or to Social Services immediately.
