WHERE CAN YOU FIND OUR POLICIES?
WHAT IF YOU GET HURT AT WORK?
1. Report the injury to your department manager as soon as the incident occurs. If your manager is not available you must go to the designated House Supervisor.
2. The manager or House Supervisor must determine whether or not medical attention is needed at this point. If an injury requires physician intervention, the employee must follow procedure by going to 1st Choice Urgent Care, or after hours by visitng our ER.
3. It is the responsibility of the employee and house supervisor to complete the incident report and to see that HR is given a copy immediately.
4. After the initial claim, any further procedures beyond normal physician follow-up visits need to be preauthorized through the Worker’s Compensation carrier to assure payment.
5. Employees who do not follow these steps and procedure may be responsible for their own medical cost or other expenses.
