WHAT DOES AN

                        APPOINTMENT LOOK LIKE?



        Patients will be required to complete an
        extensive questionnaire, which includes
      questions about birth history, family medical
     history, a personal health and lifestyle history,
     medical symptoms, diet, exercise patterns, and



     THE FIRST VISIT

     For the first visit, patients will be asked to arrive 30
     minutes prior to their appointment to get checked
     in and complete initial paperwork. This day will
     include:


       •  Office Check-in

       •  Doctor Consultation

       •  Review of Recommended Testing
       •  Lab Visit

       •  Psychosocial Consultation

       •  Review of Nutrition & Supplement
           Recommendations

       •  Scheduling Follow-up Appointments



     One week later, a patient will receive a call
     from our nurse to answer any questions and help
     facilitate the patient making the recommended
     nutritional and lifestyle changes.
     During a follow-up visit, usually eight weeks later,
     our specialists will review laboratory findings and
     positive changes already occurring. They will supply
     a detailed overview of a patient’s illness and discuss
     suggestions for further treatments and follow-up.
