Instructions for requesting MDC forms or User accounts
All MDC forms that are in production can be requested from the MDC SharePoint site below:
https://gategrouphq.sharepoint.com/teams/corp/corp/MDCPortal/SitePages/Home .aspx
Once on the site select: MDC Form Request Portal
From the MDC Form Request Portal select: New Request
From the New Form Request page, sort the catalog or search for the “gate retail” category to see the forms available.
