BENEFIT BASICS & ELIGIBILITY




         At the Down East Wood Ducks, you have access to a variety of benefits to provide financial wellness for
         you and your family. Please read this guide to learn more about your benefits, and make sure to enroll
         within  the  timeframe  allotted  by  Human  Resources  (the  first  30  days  of  employment  and  annually
         thereafter).

         The Down East Wood Ducks will provide 1095-C tax forms that include the employee enrolled in any Down East Wood
         Ducks medical plan annually.  Employees must provide family members’ name and social security number as it appears
         on their Social Security Card, if enrolled in any Down East Wood Ducks medical plan annually.
