        Associate wellbeing is critical to our success – supporting associates to be healthy,
        happy and prosperous is our goal.

           Who Can Enroll?
           •  Full-Time Associates are regularly scheduled to work at least 30 hours per week and are eligible to participate in the
              medical, dental, vision, HSA/FSA, disability, life, retirement plans and all other benefit options.

           •  Part-Time Associates are regularly scheduled to work 20 or more hours but less than 30 hours per week and eligible to
              participate in the medical, dental, vision, HSA/FSA and retirement plans, Employee Assistance Program and have access
              to free banking services.


           •  Limited Associates are regularly scheduled to work less than 20 hours per week and are eligible to participate in the
              bank’s retirement savings plans, Employee Assistance Program, and have access to free banking services.

        1  As part of the Smart90 Program, maintenance medications must be filled as a 3-month supply and must be filled at a CVS, Walgreens or ESI Home Delivery or members will pay the full cost of the medication
        and it will not apply to the deductible or OOP maximum. As part of the Diabetes Care Value Program, diabetes medications must be filled at a Walgreens Pharmacy retail store or through ESI Home Delivery or
        members will pay the full cost of the medication and it will not apply to the deductible or OOP maximum.
        2  Example: If billed rate is $1,000, contracted rate is $500, associate pays $500.
        Additional Employer Wellbeing HSA dollars available upon successful completion of company’s Wellbeing Program.
        Spousal/Domestic Partner Carve-Out – excluding coverage for spouses when coverage is available through the spouse’s employer or business.
