      We understand when you’re recovering from a hospital admission or dealing with multiple medical problems,
      you may want to relax, recover, and concentrate on dealing with your health needs. Highmark case managers are
      a free resource to Highmark members and are here to help you. With many scams and telemarketing, we want
      you to understand what case managers are and when they may call you so you don't miss an important call.


      What are Highmark Case Managers?

      Highmark case managers are licensed professionals who can offer assistance if you’re approaching a hospitalization, were recently
      discharged from the hospital or diagnosed with a condition that will require educational support, as well as help coordinate
      adminstrative aspects of your health insurance plan. Case managers can help you address financial issues, discuss caregiver needs,
      navigate community resources, coordinate a plan of care and schedule follow-up appointments if necessary.

       When Would a Highmark Case Manager Contact Me?
       If you’ve had an inpatient hospital admission, you will be contacted once or twice to make sure you’ve received and understand
       your discharge instructions, understand your prescription drugs and how to take them, and know what to expect during your
       recovery.


       What is a Conversation with a Highmark Case Manager Like?
       During the initial conversation, we complete a clinical assessment to understand your needs and tailor our interventions based on
       those needs. A case manager will work hard to establish a trusting relationship and collaborate with you to make short-term and
       long-term goals.

       Can I Reach a Case Manager Directly?
       Yes, call member service at the number on the back of your ID card and ask to speak with a case manager.
