DISABILITY INSURANCE




          Disability insurance is a very important part of the Texas Rangers benefit program. The disability
           plans are designed to provide you with a source of income if you become disabled and unable to
          work. Short term disability benefits are only payable when you are unable to work because of non-

          work-related illness or injury.  All disability leaves will run concurrent with the Family & Medical Leave

           Act (FMLA) or state equivalent where applicable.

        Short Term Disability (STD):
            •   Fully paid by the Company
            •   Benefits are:
                   o  0 to 6 months of service = No Payment
                   o  6 months to 1 year of service = 2 weeks’ pay per 12-month period
                   o  1 year to 2 years of service = 4 weeks’ pay per 12-month period
                   o  2 or more years of services = 6 weeks’ pay per 12-month period
            •   Salary  continuation  payments  will  be  based  only,  exclusive  of  overtime,  bonuses  or  supplemental  pay,  and  all
               salary continuation is contingent upon and subject to the approval of the COO.
            •   During a leave under the policy, health insurance coverage will continue.
            •   If an Employee is unable to return to work when short-term disability pay has been exhausted, the Employee may
               apply for unpaid leave status where FMLA leave has not been designated.  An Employee will be eligible to apply
               for long-term disability if unable to return to work after 90 days’ total absence.
            •   An  Employee  returning  from  a  short-term  disability  leave  must  provide  a  statement  from  his  or  her  physician
               certifying that the Employee has a release to return to work.
            •   Accruals  for  benefit  calculations,  such  as  vacation,  sick/personal  leave  or  holiday  benefits,  will  be  suspended
               during the leave and will resume upon return to active employment.

        Long Term Disability (LTD):
            •   Fully paid by the Company

            •   Has a 90-day elimination period, which means the LTD coverage will begin on the 91  day of a disability.
            •   If  an  application  is  approved  by  Unum,  an  Employee  will  receive  60%  of  their  pre-disability  monthly  earnings
               (minus any excludable earnings, such as social security payments) for the duration of his/her disability, as defined
               by Unum, or until retirement age.
