Please note that ACI decided to renew the health insurance through Aetna, new insurance carrier, as of 8/1/2014.
The application of enrollment (Includes Declining Page 5 only) needs to be submitted to me either E mail or by hand no
later than Wednesday 7/9/2014.

When we renew the policy, you may add your dependents in the policy with your own cost.
As you know, it is not allowed to add any dependent(s) after this renewal period.

The insurance agencies will be at ACI office on 7/7/2014 to assist filling out the applications and if you are in the jobsite,
you can be still getting support contacting the above listed numbers.

1. Plan Election & Rate: (Plan Aetna HMO 20, 30)
See attached plan detail
Choose either HMO 20 or HMO 30
We will send each individual rate chart for your selection

2. Application: (Aetna Application)
A late submission of application will delay all ACI enrollment process along with new I.D. card

If you apply for HMO plan, you must look for you Primary Care Physician through

Select a Plan
a. State Based Plan
b. Aetna Value Network HMO (available in CA and NV only)

4. ACI will stay same benefit coverage – HMO 30 (ACI 80% and Employee 20% )

5. Waiting Period change under the Affordable Care Act as of Aug. 1, 2014
- New Waiting Period: First day of policy month following 30 days.
