The Annual Health Insurance Open Enrollment has just begun!
At Angeles Contractor, Inc., we recognize the hard work and dedication that goes into your work every day.
That’s why as an Angeles Contractor, Inc. employee, you have access to a comprehensive benefits package.
During Open Enrollment, reflect upon how your life has changed over the past year and consider how it may
be different next year. This is the opportunity to enroll yourself or add family members in plans sponsored by
Angeles Contractor, Inc. You can also make needed changes such as adding spouse and/or children by
completing and returning the attached Medical, Dental & Vision Enrollment/Change form to HR no later

than July 14 , 2017. New enrollments and changes will take effect August 1, 2017. Please note that if you do
not enroll this time, you may not be able to enroll until August of next year unless you have qualifying
event. If you don’t make any change this time, you may default into the same or comparable coverage that
you elected last year.
Angeles Contractor, Inc. is delighted to announce that we will change our Vision Carrier to Humana effective
August 1, 2017.
