Please note that the latest Health Insurance coverage of benefits and ACI Human Resources distributes
the Employee Benefit Handbook (2017) as attached.
The purpose of the Employee Benefit Handbook is to provide information and guidance to all employees
to find a right plan and understand your plan.

The 53 page booklet includes Benefit eligibility, benefit summaries, rate, and how to find a doctor in your area.
The contact information for Insurance agency, Chun‐Ha Insurance and each carrier’s member service contacts
are attached for you to contact whenever you need assistance. It also contains how to log in to member
portal.
