MEDICAL PLAN CHOICES

Angeles Contractor, Inc. offers regular, full-time employees
and eligible dependents a choice of two medical plan
options:

HMO 1
UHC HMO Advantage Platinum 20-40/20% (BR-SX)

HMO 2
UHC HMO Advantage Gold 30-60/1000 (BH-GZ)

Employees may choose one of these two plans.
You are required to pay a portion of the premium for
medical plan.
