Safety Memo #36 – City of Los Angeles Vaccination Proof Requirement

[ City of Los Angeles Vaccination Proof Requirement ]

The City of Los Angeles passed a new order that took effect on Monday, November 8, 2021. This order requires patrons
of some Los Angeles businesses to show proof of vaccination when they enter. The following indoor locations will now
be affected:
• Food and beverage establishments (restaurants, coffee shops, food courts, etc.)
• Gym and fitness venues
• Establishment and recreation venues (malls, movie theaters, live performance venues, etc.)
• Personal care establishments (hair salons, barbershops, spas, etc.)
• Indoor city facilities (city government buildings)

Here are the acceptable forms of proof of vaccination:
• A vaccination card which includes your name, type of vaccine provided, and the date of the last dose
administered
• A photo of a vaccination card (both sides)
• Documentation of vaccination from a licensed healthcare provider
• A personal digital COVID‐19 vaccine record issued by the State of California or similar documentation issued by
another state, local, or foreign governmental jurisdiction, or by a private company

*California residents can visit the State of California’s Digital COVID‐19 Vaccine Record website at
https://myvaccinerecord.cdph.ca.gov/ and download a QR Code on a phone.
