Disability Coverage:

Long-Term Disability Coverage
SIPS Consults, Corp. provides employees long-term disability coverage to protect their income.
• Benefits begins after 90 days following an illness or injury
• Benefits can continue up to Age 65 or Normal Social Security Retirement Age
• Benefit Amount pays up to 60% of your income with an additional Progressive Income Benefit of
10% for a Total Available Benefit of 70% based on your income loss and/or definition of disability
• Maximum Monthly Benefit paid up to $7,500 based on Income
• Under the 10% Progressive Income Benefit (PIB), the Maximum is up to an additional $5,000
• Your policy pays for both Total or Partial Disabilities caused by Accidents and Illness
DEFINITION OF DISABILITY:
Total Disability: Due to an injury or illness, you are unable to perform each of the main duties for 24 months of
your own occupation. Following this, the definition of total disability becomes the inability to perform any
occupation for which you are reasonably suited based on your experience, education, or training.
Partially Disabled: Due to an injury or illness, you are unable to perform one or more of the main duties of your
regular occupation on a full-time basis. Partial Disability benefits may be payable if you are earning at least
20% of the income you earned prior to becoming disabled, but not more than 99%. Partial disability benefits
allow you to work and earn income from your employer as well as continue to receive benefits, which may
enable you to receive 100% of your income during your time of disability.
Progressive Income Benefit (PIB): The Progressive Income Benefit, which is standard on our LTD plans, automati-
cally increases the core benefit by 10 percentage points when an insured employee suffers a serious cognitive
impairment or the loss of two or more Activities of Daily Living (ADLs): bathing, dressing, using the restroom,
transferring (to and from a bed, chair or wheelchair), continence or eating.”
Pre-Existing Condition, means any sickness or injury for which you received medical treatment, consultation,
care, or services (including diagnostic measures or the taking of prescribed medications) during 12 months
prior to your coverage effective date. A disability arising from any such sickness or injury will be covered only
if it begins after you have performed your regular occupation on a full -time basis for 12 months following the
coverage effective date.
Coverage will terminate when you terminate employment with this policyholder, or at your retirement.
OTHER BENEFITS:
Rehabilitation Assistance Benefit: Employees who participate in an approved rehabilitation program are
eligible to receive an additional percent of benefit. Additionally, approved program costs may be
reimbursed.
Waiver of Premium: You will not be required to pay premium during any time of approved total or partial disa-
bility.
Survivor Income: A benefit may be paid to your survivor for additional months if you should die while you
were eligible to receive benefits under this policy.
