         Voluntary Long Term Disability:





                                          Long Term Disability Coverage

           TriTrax Rehabilitation provides employees with long term disability coverage intended to protect
           your income in case you become ill or injured.

            Benefits begin after 90 days after an injury or illness

            Benefits can continue up to age 65 or Normal Social Security Retirement Age

            Benefit amount pays up to 60% of your income
            Maximum Monthly Benefit paid is $6,000


            Pays for Total and Partial Disabilities caused by accidents and sickness
           DEFINITION OF DISABILITY

           Total Disability:

            The Elimination and Own Occupation Period means that due to an injury or illness, you are
              unable to perform at least one of the main duties of your own occupation during the Own
              Occupation Period.

           Partially Disabled:

            Due to an injury or illness, you are unable to perform one or more of the main duties of your
              regular occupation on a full-time basis. Partial disability benefits allow you to work and earn
              income from your employer as well as continue to receive benefits, which may enable you to
              receive 100% of your income during your time of disability.
