TriTrax Rehabilitation is committed to a comprehensive benefit program that
helps our employees stay healthy and feel secure while maintaining a work/
life balance.
TriTrax Rehabilitation is pleased to offer dual medical options, voluntary-
dental, vision, life and disability coverage to our employees and their
dependents. We also continue to provide 100% employer paid life and
AD&D for our employees.
TriTrax Rehabilitation covers 50% of either medical plans for the employees
only premium to make this more affordable.
Please retain a copy of this guide in order to review coverage benefits, cost
information, and necessary contact information on page 19. This booklet will
not serve as an ID card or verification of benefits at a provider’s office, but
can provide assistance if needed. For more information and to review the
Glossary of Health Coverage and Medical Terms, you can log into the TriTrax
