Stamford Residence and Rehabilitation Center is pleased to offer our full-time employees four Unit-
ed Healthcare major medical plan options and three Pan American plan options with that offers
Unlimited annual preventative care benefits with NO out of pocket costs. Both plans offers Virtual
Healthcare visits at $0 Copay cost. Voluntary Dentary is offered thought Delta Dental and a NEW
vision program with enhanced benefits Superior Vision.

Stamford Residence and Rehabilitation Center pays a major portion of the employees cost for any
of the medical options you choose.

Please retain a copy of this guide in order to review coverage benefits, cost information, and retain
necessary contact information. This booklet will not serve as an ID card or verification of benefits at
a provider’s office, but can provide assistance if needed. Refer to page 22 for instructions on how
