            Citizens Bank of Ada is committed to a comprehensive benefit program that helps our
            employees stay healthy and feel secure while maintaining a work/life balance.

            Citizens Bank of Ada is pleased to meet your individual needs by offering you medical,
            dental, vision, basic life and AD&D, long-term disability and voluntary life and AD&D.
            Citizens Bank of Ada’s medical plan also offers a 24/7 Telemedicine benefit through
            Teladoc. We still offer an Employee Assistance Program (EAP) for all employees and
            dependents.

            Citizens  Bank  of  Ada  pays  the  majority  of  the  cost  of  the  employee’s  medical  and
            100% of dental based on your medical election. Our employees also have the option
            to  purchase  dental,  vision,  and  voluntary  life  coverage for  themselves,  their  spouse,
            and children without medical. We pay 100% of your long-term disability and basic life
            and AD&D. Our employees still have the option of guaranteed issue amounts this year
            for voluntary employee life of $100,000, voluntary spousal life of $30,000, and voluntary
            child  life  of  $10,000.  Current  covered  employees  under  voluntary  life  are  able  to
            increase up to $10,000 without health questions.

            contact information.  This booklet will not serve as an ID card or verification of benefits

            at a provider’s office, but provides a basic summary of benefits available to you. For
