Assessment and evaluation of students (old & new) during enrollment for the 2
semester SY- 2019-2020

The enrollment was scheduled January 7-11, 2020 but it extended until the end of
January

 Creation of individual health record for new students, assessment of
medical history, and vital signs taking
 Orientation of the internal processes and operations in the clinic
 For old students, update the individual health record
 Referral to higher health facility is also done if necessary especially to those
evaluated with health problems and complications, and required medical
certificate upon return to ensure safety of the university
