activities. Requests for changes are to be coursed through the Office of the
Vice President lor Academic Affairs for action by the President.
3 Faculty Evaluation by the supervisors are to be done by the following

2.2.Deans for Chairpersons
2.3.Campus Executive Directors for Deans
2.4.Vice President for Academic Affairs for Campus Executive Directors
4. Evaluation of faculty members by the students shall be administered by the
guidance offices of aU campuses.
