Life and Accidental Death & Dismemberment Insurance

Life Insurance
Life insurance provides financial security for the people who depend on you. Your beneficiaries
will receive a lump-sum payment if you die while employed by Burke. The company provides basic life insurance of one times your annual salary up to a maximum of $50,000 (provided at no cost to you).
Accidental Death and Dismemberment (AD&D) Insurance
Accidental Death and Dismemberment (AD&D) insurance provides payment to you or your beneficiaries if you lose a limb or die in an accident. Burke provides AD&D coverage of one times your annual salary up to a maximum of $50,000. This coverage is in addition to your company-paid life insurance described above and is provided at no cost to you. If you are interested in updating your beneficiaries on your Life Insurance Policy, please contact Human Resources.
Voluntary Life and AD&D Insurance
Insured by First Reliance Standard Insurance Co.
Employees who want to supplement their Burke
paid group life insurance benefits may purchase additional coverage through First Reliance Standard’s voluntary program. When you enroll yourself and/or dependents into this program, you are responsible for paying the full cost through payroll deductions.
Employee Amount: Employees may elect increments of $10,000 to a maximum of 5x annual salary or $500,000 (the lesser of).

Spouse Amount:
50% of the Insured’s Supplemental Life Amount, subject to a maximum Insurance of $100,000
Child Amount:
Live birth to 6 months: $500 6 months and over: $10,000
Burke employees who waive Voluntary Life Insurance upon hire and would like to purchase additional coverage in the future MUST complete an “evidence of insurability questionnaire”.
