VOLUNTARY AD&D
You may purchase supplemental life insurance and voluntary AD&D coverage to provide you and your family with additional nancial security. Life insurance is about more than paying for memorial services- it’s about making sure
your family can maintain its standard of living over the long run if something happens to you. How much your family needs depends on your personal situation (other income, monthly expenses, and short-and long-term debt).
» You can elect Supplemental Life and Voluntary AD&D insurance coverage for:
» Yourself: Increments of $10,000 up to $500,000. You are guaranteed up to the following speci c amounts based on your age, without submitting EOI: Under age 60: $50,000 /Age 60 but under age 70: $10,000 / Age 70 or older: none
» Your Spouse: Increments of $10,000 up to $500,000 elected amount. Your spouse is guaranteed up to the following speci c amounts, based on age, without submitting EOI: Under age 60: $50,000 / Age 60 or older: none
» Your Child(ren): Birth to 6 months: $1,000 / Age 6 months to 20 years of age (26, if full-time student): choice of $10,000
You pay 100% of the cost for this coverage. Please refer to the plan summaries for age-related rates.
DID YOU KNOW?
If you did not enroll in this program when you rst became eligible, and wish to elect coverage at a later date, you will be required to provide Evidence of Insurability to the insurance company.
