Wellness Incentive
New Incentive Bonus!
All bonus activities listed below must be completed January – December.

and options for your wellbeing and are
pleased to announce our Improved
Wellness Incentive Program! For the
upcoming year, each Wellness Activity has a percentage value, and upon completion of each activity you earn points. You could earn a reduction in your medical premium contributions of $500 annually
or $19.23 per pay period by participating. To be eligible for the reduction, you must complete 4 out of 5 activities for a total of 100 points. The Improved Wellness Incentive Program runs from January 1st through October 31, 2022.
We hope you engage in these simple tools and valuable offerings toward a healthier lifestyle.
Complete 4 out of the 5 Wellness activities listed below:
1. Complete a Biometric Screening (25%)
2. Complete Rally Health Survey on the United HealthCare website (25%), www.myuhc.com.
3. Complete your annual physical (25%)
4. Complete your annual Dental visit (25%)
5. Gym Check in (5% per month)*

2. Complete a Wellness Coaching Program ($50.00)
3. Get a Flu shot ( $50.00)
4. Attend a Health Fair ($50.00)
Compliance will be tracked by UnitedHealthcare. To ensure privacy, please do not submit any personal information/medical records
to the Benefits Department. The wellness activities only apply to Service Corporation employees, spouses and dependents are not included.
Upon completion of each bonus activity, you will be eligible to select a gift card of your choice.

Disclaimer: This program is HIPAA compliant. The Voluntary Wellness Incentive Program is only available to employees participating in the United HealthCare medical plan. Employees must be active for the 2022 plan year in order to be eligible for the reduction.
