Medical Benefits:
Effective 1st of the month following 30 days of employment.
We currently offer (2) medical/prescription drug plans with United Healthcare, Choice Plus POS and Choice EPO HRA. With either plan you can receive care from the provider of your choice without ever needing to select a primary care provider (PCP) or obtain a PCP referral for specialist care.
Provider searches can be done at www.myuhc.com. Use the myuhc.com® website or app and simplify your search for providers. Easily find providers in your plan’s network and compare costs for hundreds of procedures.
Eligible dependents are defined as follows: A legal spouse and any child who has not reached age 26, and who is chiefly dependent upon the employee for support.
The term “child” means the employee’s children, including any legal stepchild, adopted child, or child for whom the employee or employee’s spouse is the court-appointed legal guardian. A prospective adopted child is eligible for coverage on the same basis as a natural child during any waiting period prior to the finalization of the child’s adoption.
