        Availability of Summary Health Information

As an employee, the health benefits available to you represent a significant component of your compensation package.
They also provide important protection for you and your family in the case of illness or injury. As a plan participant, you
are entitled to a comprehensive description of your rights and obligations under the McLennan County Employee Health
        Plan.

Your plan offers health coverage options. Choosing a health coverage option is an important decision. To help you
make an informed choice, your plan makes available:

            • A Summary Plan Description (SPD)
            • A Summary of Benefits & Coverage (SBC)

The SBC summarizes important information about any health coverage option in a standard format, to help you
compare across options. In order to ensure that you fully understand the benefits available to you and your obligations
as a plan participant, it is imperative that you familiarize yourself with the information contained within the SPD.

The SPD & SBC are provided to you on the McLennan County Intranet under Human Resources, or on the S&W website
https://mclennan.swhp.org/. If you would like to receive a paper copy of these documents, please contact Human
Resources and one will be provided to you free of charge.
