Pemi-Baker Hospice & Home Health Offers Complex Care Support for Common Serious Illnesses of Aging

As people age, many begin to experience chronic health con- ditions that can make everyday life more complicated. Illnesses such as cancer, heart disease, Alzheimer’s disease, COPD, kidney disease, stroke, and dia- betes are increasingly common among older adults, and manag- ing them often requires ongoing care, education, and support.
To help patients and families navigate these challenges, Pemi- Baker Hospice & Home Health offers its Complex Care Support Program, designed to provide additional assistance to individ- uals living with serious illnesses while they continue receiving active medical treatment.
For many families, managing a serious health condition can quickly become overwhelming. Multiple doctor appointments, medications, changing symp- toms, and uncertainty about what lies ahead can create stress for both patients and caregivers.
“Many of the health chal-
lenges people face later in life are a normal part of aging, but that doesn’t mean families have to navigate them alone,” said Aubrey Engle, RN, Executive Director at Pemi-Baker Hospice & Home Health. “Our Complex Care Support Program provides an extra layer of guidance and care so patients and caregivers feel more confident managing these conditions at home.”
The program focuses on education, care planning, and resource navigation, helping patients better understand their illness and the options available to them.
Pemi-Baker’s skilled home health team includes experi- enced registered nurses, licensed nursing assistants (LNAs), physi- cal therapists, and occupational therapists who work together to support patients in the comfort of their homes.
Services through the Com- plex Care Support Program may include:
• Nursing visits to monitor
symptoms and help manage
complex medical needs
• Medication education and management to ensure treat-
ments are safe and effective
• Physical and occupational therapy to maintain strength,
mobility, and independence
• Personal care assistance pro- vided by compassionate li-
censed nursing assistants
• Education for patients and caregivers so families feel pre-
pared and informed
• Care planning and guidance
to help patients and families understand what to expect in the future
By offering care in the home, the program helps individuals remain in familiar surroundings while receiving the support they need to maintain the best possi- ble quality of life.
A key part of the Complex Care Support Program is collab- oration with existing healthcare providers. The Pemi-Baker team works in conjunction with physi- cians and specialists, enhancing the care patients are already re- ceiving and ensuring communi-
cation between all members of the care team.
The program also offers nurses on-call 24 hours a day to assist with urgent needs, pro- viding reassurance that help is always available if questions or concerns arise.
“Serious illness often comes with uncertainty,” Engle added. “Having a knowledgeable team available at any time can pro- vide tremendous peace of mind for families.”
Beyond addressing immedi- ate needs, the Complex Care Support Program also helps pa- tients and families prepare for what may come next, empower- ing them to make informed de- cisions about their care as health needs change.
