Care Plan MOBILITY
From CO LTC Licensure regs: PROMOTION OF MOBILITY
• (1) For all residents, the facility shall have assessed each resident's ambulation potential and capability at least monthly, designed a plan of care as part of the care plan prescribed in Section 5.7 to encourage mobility, be implementing the plan, regularly evaluate progress and revise the plan as needed.
• (2) For residents requiring devices and/or personal assistance to ambulate, the facility shall provide and maintain devices in good repair, assist the resident to obtain appropriate footwear, and provide assistance to residents to move and transfer.

GROW: Getting Residents Out of Wheelchairs
• Training for all staff/disciplines
• Assess why a resident is in the wheelchair in the first place
• Plan what all disciplines can do to help walk once again
• Daily prescribed exercises to assist in reaching the highest practicable
physical, mental and psychosocial well-being as required
• Provide productive restorative care that is Medicare reimbursable under restorative care.
• Replace the fall prevention mind set with an ambulation independence perspective to be accomplished through daily exercise programs led by trained restorative CNAs
• Falls will not be as injurious as residents are stronger
• Pressure ulcers practically disappear as people who walk don’t get
them
• All staff contributes to preventing falls
