2017 Employer Paid Coverages

Basic Life and Accidental Death and Dismemberment

• QuesTec provides a $20,000 Basic Life and $20,000 Accidental Death and Dismemberment
policy for all full-time employees while employed.

Short Term Disability (STD)

• Short-term disability is an employer provided benefit for a full-time employee while employed at
QuesTec.

• STD benefit covers 60% of your salary to a maximum of $750 per week for 11 weeks.

• The benefit starts on the 14 day of disability for illness, and 14 day for an accident.

Long Term Disability (LTD)

• Long-term disability is an employer provided benefit to cover until age 65 – standard ADEA for a
full-time employee while employed at QuesTec.
• LTD benefit covers 66.67% of your salary to a maximum of $6,000 per month.
• The benefit starts on the 90 day of disability.
