P. 65

Employees who wish to take time off for parental leave (not otherwise related to pregnancy or maternity leave) will be required to use all available paid time off to run concurrently with any eligible FMLA leave benefits.
During maternity and parental leave, all benefits provided under an employee benefit plan are governed by the terms and conditions of the applicable employee benefit plan documents in accordance with applicable law. For all other benefits, an employee on maternity or parental leave will receive the same rights and benefits as employees on FMLA leaves of absence.
10-10. Unpaid Leave of Absence
Should a situation arise that temporarily prevents an employee from working, the employee may be eligible for an unpaid leave of absence.
Any request for an unpaid leave of absence must be submitted to the Director of Human Resources in writing as far in advance as possible. Human Resources, in cooperation with the employee’s manager, will evaluate requests for unpaid leave on an individual basis and notify the employee in a timely manner whether or not the request has been granted.
Employees may only request an unpaid leave of absence after all vacation and sick time have been exhausted and no FMLA leave is available. The duration of an unpaid leave of absence, if granted, will be contingent on the specific circumstances of each individual employee requesting the unpaid leave. While on an unpaid leave, the employee’s group health benefits will be continued for the duration of the approved leave. During an unpaid leave of absence, the employee will not accrue vacation or sick time. Further, if the unpaid leave concerns the employee’s own medical condition, the employee will be required to provide Erbe with a release to return to work and any restrictions following the employee’s return to work.
Following return from an unpaid leave of absence, unless otherwise required by federal or state law, the company will attempt to reinstate the employee to his/her former position or to one with similar responsibilities.
