fatal and nonfatal injuries among people aged 65 and older. We are offering a [free/low-cost]
fall prevention program to individuals whose current health status places them at increased
risk of falling. Please recommend our service, described in more detail below, to your patients
who would benefit from it.
Our program is [name and description of program; program details. For example:
“Tai Chi: Moving for Better Balance,” an evidence-based program for older adults at
risk of falling due to leg weakness and poor balance.
We will hold twice-weekly Tai Chi classes, led by a trained Tai Chi instructor, beginning March
