• Full-time therapists are expected to attend mandatory meetings, including staff meetings and case conferences. Every effort will be made to keep these meetings within normal business
hours, but the times and/or dates of these staff meetings may fall outside normal business hours.
• False representation of the reason you are absent from work may lead to disciplinary action up to and including termination.
Because the company depends on its employees to be at work at their scheduled times, excessive tardiness and absenteeism may lead to disciplinary action up to and including termination. The determination of excessive tardiness and absenteeism will be made at the discretion of the company.
If you are absent without notice for three days in a row, you will be considered as having abandoned your job, and the Company will process your work separation as a voluntary resignation on your part.
BACKGROUND CLEARANCE POLICY
Circle of Care Human Resources department shall conduct a criminal history check and/or background check on all applicants for employment to include patient caregivers and office personnel. Employment is contingent upon a successful criminal history check in accordance with State licensing entity guidelines and agency policies.
Circle of Care will not employ any person who has been convicted of a felony or misdemeanor involving any type of sexual contact with a child, or abuse of a child (including volunteers). This includes, but is not limited to indecency with a child, injury to a child, or endangerment of a child. A conviction includes deferred adjudication.
BENEFITS
Circle of Care offers all regular full-time employees the following benefits:
• For full-time therapists to be eligible for insurance benefits, federal law requires maintaining a minimum average of a 30-hour work week.
• Paid time off each year, pro-rated & granted according to agency policy.
• Employer paid 100% monthly base premium for employee coverage for medical insurance (buy up options available); employee will be responsible for all dependent premiums. Employee is
eligible for these benefits after 60 days of employment.
• 401K Plan is provided and is solely funded by employee contribution. You are eligible to
participate on the first day of the quarter, following 60 days of employment (this benefit is
available to full-time and to part-time employees).
• Optional Supplemental Benefits such as Dental, Vision, Short-Term Disability, Voluntary Life,
and a Basic Hospital Indemnity plan. Employee is responsible for supplemental benefit premiums.
BEREA VEMENT
Full-time employees are eligible for up to 3 days of paid bereavement time for the death of an immediate family member. For purposes of this policy, an immediate family member is defined as an employee’s: spouse, domestic partner, mother-in-law, father-in-law, child, parent, grandparent, grandchild, sibling, stepchild or stepparent. Additional time off may be requested with the use of PTO or an unpaid leave approved by the company.
