 CHANGE IN EMPLOYMENT STATUS
As a full-time therapist, you must meet a minimum threshold of visits or units required to meet productivity standards and submit clinical documentation according to guidelines to maintain a full-time position and retain full-time company benefits.
The company reserves the right to change a therapist’s status from salaried to PRN and/or from PRN to part-time, at the company’s discretion. A change to part-time status will result in the loss of all company benefits. A change to PRN status may result in the loss of some or all company benefits. If a change to part-time and loss of benefits does occur, the employee must wait 60 days before consideration of benefit restoration to employee.
For purposes of this policy, PRN is defined as a full-time paid per visit or paid per unit employee.
CHILDREN IN THE WORKPLACE
Employees are responsible for verifying with their manager the circumstances under which children are allowed in their specific workplace.
Management may revoke previously granted permission for the employee to bring the child to the workplace for reason determined by the manager.
CLINICAL DOCUMENTATION
Non-compliance with this policy could result in disciplinary action including formal written warning placed in your employment file, monetary penalties, change of employment status (for example, salaried to pay-per-visit or fulltime to part time) and termination of employment.
CLINICAL DOCUMENTATION - TIMELINE
Clinical documentation to include but not limited to daily visit notes, progress notes, evaluations and re-evaluations will be completed according to the following timeline:
• Check in all patient contact visits on the same date as the visit.
• Complete and sign off on daily notes within 24 hours of the visit.
• Complete and sign off on progress notes, evaluation and re-evaluation reports within 72 hours of the visit.
CLINICAL DOCUMENTATION - SUBMISSION OF LATE NOTES
Clinical documentation completed and signed off after the final day of the care week, Sunday evening at midnight, may be subject to penalties as stated below:
• PRN (pay per visit) employees may not be paid in the upcoming pay period if the notes for
that period are not turned in by the deadline of midnight the Sunday before the pay period.
• ALL notes submitted after the Sunday midnight deadline of each care week may incur an
auto-deduction of $8.00 per note.
• Salaried Therapists demonstrating consistent delays may be converted to PRN (pay per visit)
and late notes/evaluations will be subject to a deduction of $8.00 per note submitted after the deadline. Therapist must notify payroll when/if completion is delayed.
