• The only exception to the above Sunday at midnight deadline is for any progress note, evaluation or re-evaluation that was completed on the Friday, Saturday, or Sunday before the Sunday deadline. These progress notes, evaluation or re-evaluation reports are due by following Tuesday at 12:00pm / Noon. (If a holiday falls during a payroll week, the deadline for the above exception is Monday at 12:00pm/ Noon.) Therapist must notify payroll when/if completion is delayed.
• Continued submission of late notes and non-compliance with this policy could result in further disciplinary action up to and including termination of employment.
DEADLINE REGARDING CASELOAD SCHEDULES
• Schedule MUST be completed in Raintree by 8:30 am on Friday of each week for the upcoming treatment week.
• Continued non-compliance with this policy could result in disciplinary action up to and including termination of employment.
DISCIPLINARY ACTIONS
Circle of Care may terminate for cause and/or utilize a progressive discipline system. While it is generally desirable to follow these steps, the agency reserves the right to skip some or all of the steps up to and including termination:
1. Verbal Warning
2. Written Warning
3. Final Warning
4. Termination
Reasons for disciplinary action up to and including termination may include, but are not limited to:
• Theft of company or personal property.
• Demonstrating a lack of respect for following guidelines and rules/authority.
• Unacceptable job performance.
• Excessive absences and/or tardiness.
• Excessive personal internet or phone usage.
• Use or selling of drugs or intoxication.
• Harassing other employees/clients, either verbally or physically.
• Any action in conflict with stated company policies.
• Sexual misconduct to include but not limited to: patients, patient’s family members or
agency employees.
   CONFIDENTIALITY AGREEMENT POLICY
 Circle of Care has a confidentiality agreement policy that is a condition of employment. This policy is in place to protect the company as well as its staff and clients from being used or coerced to provide assistance or information to an agency in direct competition with Circle of Care. All employees are required to sign a confidentiality agreement.
