 EMPLOYMENT CLASSIFICATIONS
Upon being hired by the company, employees must complete a sixty (60) day probationary period. Your performance will be carefully monitored during this period (and extended based on performance). Satisfactory completion of the probationary time does not entitle you to any specific term but does enable you to participation in many of the company’s employee benefit programs.
1. Regular Full-Time – An employee who is scheduled to work an average of forty (40) hours per week on a regular and continuous basis OR has an employment agreement to do a minimum number of visits/units per week.
2. Regular Part-Time – An employee who is scheduled to work less than an average of thirty (30) hours per week on a regular and continuous basis OR is paid only on a per visit basis and does not have a minimum visit requirement per week. (SLPAs are required to have at least 12-15 visits per week.)
For payroll purposes, employees will be classified as one of the following:
1. Exempt – Certain employees such as executive, administrative, professional and marketing are paid on a salary basis for all hours worked each week. These employees are expected to work the hours required to accomplish their duties, even if it exceeds their normal workweek. No overtime pay will be paid to exempt employees.
2. Non-Exempt Hourly: Paid at a per hour rate, eligible for overtime pay. Overtime must be pre- approved by the immediate supervisor. Work time must be documented by clocking in / out daily through Proliant.

FMLA LEAVE
Employees who have been employed for at least one (1) year AND have worked at least 1,250 hours during the preceding 12-month period are eligible for family and medical leave. This leave consists of accrued paid leave and unpaid leave. Please contact Human Resources for more information on the FMLA.
FRAUD STATEMENT
Circle of Care strictly forbids fraudulent altering or tampering of therapy or medical notes. It is also illegal to deliberately misrepresent or falsify information on a patient’s notes, charts, evaluations or plan of care. Violation of this rule will result in immediate termination and possible legal recourse.
