 NOTE HANDLING
All therapists are required to obtain the caregiver’s electronic signature acknowledging each visit. If electronic signature is unable to be obtained, handwritten signature log is acceptable. Copies of notes can be provided to the patient’s parent/caregiver upon request. Incorrect notes or notes missing vital information will not be considered complete until a corrected copy has been updated in the EMR system.
ON CALL SERVICES
On call services are available 24 hours a day, 7 days a week, including after business hours, weekends, and holidays.
PAID TIME OFF (PTO)
PTO
Circle of Care offers paid time off (PTO) to its full-time employees (the equivalent of 40 hrs per week per ACA guidelines), dependent upon job classification and maintenance of full-time hours. PTO may be used for personal time, vacations or sick time.
PTO - Conversion of Visits and Units
For non-exempt employees; (i.e., all assistants) & exempt employees receiving a base salary plus, per visit/unit pay to reach standard productivity as agreed upon by employee/employer.
A. PTO is calculated based on actual gross productivity of Units/Visits over the previous 90-day look back period. (PTO pay will not exceed 100% of your standard expected productivity per pay period: 60 visits for SLPA or 180 Units for OT/OTA, PT/PTA).
B. For Exempt Employees, PTO is calculated based on the employee’s standard rate of compensation.
PTO - Deposit for Employees
PTO will be credited on an annual calendar-year basis on January 1st of each year. For newly hired employees attaining full-time status, PTO is credited for use in the first full pay period of the month after 60 days of employment.
PTO – Method of Deposit for newly hired, full-time employees hired within the calendar year
Upon hire, the employee will be granted a pro-rated amount of PTO depending on date of hire.
PTO - The following rules will apply:
1. PTO may not be used until the employee has successfully completed the 60-day probationary period.
2. Use of PTO must be requested in the Proliant system at least two weeks in advance. Permission is granted at the Company’s discretion based upon staffing and business needs.
3. As a rule, PTO is not granted for two consecutive weeks of vacation. For a two-week request to be considered through the approval process, the therapist must arrange to have their potential visits covered in their absence and submit this coverage with the PTO request.
