In the Hierarchy Type field,
click the drop down and
select “Organization” from
the drop-down menu

**Do not type the clinic
name or number in the
“Organization” field.

4 To choose a clinic, click the
magnifying glass A pop-up
with a hierarchy menu will
appear (shown to the right)
Follow the hierarchy until
you find the desired clinic
Finish by clicking “Select” at
the bottom of the dropdown
The selected clinic will
populate in the Organization
field
