4 In the Hierarchy Type field,
select the drop-down and
select Organization from the
drop-down menu

**Do not type the clinic
name or number in the
Organization field.

5 To choose a clinic, select the
magnifying glass A pop-up
with a hierarchy menu will
appear (shown to the right)
Follow the hierarchy until
you find the desired clinic
Finish by choosing Select at
the bottom of the dropdown
The selected clinic will
populate in the Organization
field
