                                                           Eligibilty

                                       You are eligible to participate in Medical, Dental, Life
                                       Insurance, STD, LTD, 401(k) and FSA benefits if you are a
                                       full-time or part-time employee regularly scheduled to work
                                       30 or more hours per week.  When you enroll yourself, you
                                       may also cover your eligible dependents for Medical,
                                       Dental, Vision and Life Insurance coverage.
                                       Part-time employees who are scheduled to work less than
                                       30 hours are eligible for 401(k) contributions and prorated
                                       PTO benefits.
                                       All staff are eligible to receive assistance from our EAP
                                       (Employee Assistance Program).
                                                    Change In Family Status?

                                       Your benefit selections remain in effect until the end of the
                                       plan year.

                                       You may change your benefits during the year only if you
                                       have a change in family status (marriage, birth, adoption,
                                       divorce, etc.  See your HR representative for a full list of
                                       Qualifying Events.)

                                       You have 31 days from the date of the status change to
                                       contact Human Resources and make any change to your
                                       benefits elections.
                                                        Waiting Periods

                                       Medical, Dental, Vision, STD, Life, etc. are effective the first
                                       day of the month following 60 days of Full-Time
                                       employment.
                                       Employees can begin using their paid time off (PTO) after
                                       90 days of employment.
