Identification Cards
You will receive identification cards when you enroll for the benefits listed below.
If you enroll in the Medical/Prescription Drug plan
offered at Hospice & Palliative CareCenter, you will
Medical/Prescription Drug receive an ID card from MedCost. You may request
additional cards by contacting MedCost customer
service at 1-800-824-7406.
If you enroll in the Dental plan offered at Hospice &
Palliative CareCenter, your plan member ID card is
provided electronically. Access your eCard online by
Dental creating a secure member account at Ameritas.com.
Enrolled members may receive care without the card
just by giving the provider their name, date of birth,
and social security number/member ID number.
If you enroll in the Voluntary Vision plan at Hospice &
Palliative CareCenter, you will receive an ID card from
Community Eye Care. You may also access a copy of
Voluntary Vision your card by visiting the ‘Members Portal’ online at
cecvision.com. You may request additional cards by
contacting Community Eye Care customer service at 1-
888-254-4290.
If you enroll in the health care FSA plan at Hospice &
Palliative CareCenter, you will be sent a debit card to
Flexible Spending Accounts
be used on qualified health expenses. You may access
your account information online at Flores247.com.
