Identification Cards
You will receive identification cards when you enroll for the benefits listed below.
If you enroll in the Medical/Prescription Drug plan
offered at Members Credit Union, you will receive an ID
Medical/Prescription Drug card from United Healthcare. You may request
additional cards by contacting United Healthcare
customer service at 1-800-842-8000.
If you enroll in the Voluntary Dental plan offered at
Members Credit Union, you will receive an ID card from
Voluntary Dental Unum. You may request additional cards by contacting
Unum customer service at 1-866-679-3054.
If you enroll in the Voluntary Vision plan at Members
Credit Union, you will receive an ID card from Blue 20/20
Voluntary Vision with EyeMed. You may request additional cards by
contacting Blue 20/20 with EyeMed customer service at
1-855-400-3641.
