I care, so I have decided to assist in bringing Awareness about Alzheimer’s. However vast, the
areas of Awarness span, I endeavor to focus on the Care that is rendered to Alzheimer’s Patients.
I want to use my resources in developing ways to provide Support and Incentives to Care Givers
who are Professional staff at facilities throughout the nation.
My Goal:
To establish a (Pilot) Incentive Program, by September 1, 2016 to reward Caregivers who provide for
Alzheimer’s Patients. After a 6 - 12 month evaluation, if the Pilot Program is successful, I would
like to explore ways to implement it in more facilities in various states.
Action Steps:
Work with Various Senior Care Faclities to identify 4 Locations in Southern California to
participate in an (pilot) Incentive Program that rewards CAregivers with various Gifts and
Prizes for being winners of the “Caring Heart” Incentives Program.

-How it works
each caregiver, (all employees excluding managers and administrative staff)
will have an opportunity to be nominated by their working peers, family members of the
Patients/Residents, managers and administrative staff for providing exceptional care.
Nominations and votes will be tallied and rewards will be given to the winners each month.
This will be an app based nomination process which will be easy to submit, but the person
submitting the nomination will need to provide a brief explanation for the nominee.
-Types of Rewards and Incentives*
Gift Cards, Apparel and accessories (with incentive program branding), Gift Baskets,
Movie Tickets

*I plan to meet with governing bodies of other agencies that either have an incentive progam
or has knowledge of Policies and procedures for establishing a program such as this. Also
I will consult the IRS and an Accountant to determine the type of taxation impact that
receiving Incentives could have on the recipient.
