Initial Skills Validation Checklist
As an employee of Piedmont Healthcare, it is expected that the employee will complete the Initial Skills Validation Checklist during the
specified orientation time. These skills are determined based on job roles and department needs. Each skills checklist packet is specific
to the division for which it is labeled. These skills are to be discussed and completed with the employee and a qualified assessor. The
qualified assessors can be managers, supervisors, educators, shift managers, charge nurses, preceptors, super users, or staff deemed
competent to assess new employees. Please discuss with the management team of the department who is qualified to check off new
employees.
The skills Validation Checklist is to be completed during orientation and is expected to be completely filled out at the end of orientation.
Successful completion of this checklist will be required to work independently. Please discuss with the management team before
coming off orientation if this is not a possibility.
An example of a completed skills checklist page is provided on the next page of this packet. Each box should be filled in, not leaving
any box blank. If the skill is not applicable to the department, the employee may write N/A in the box. Each box should be filled out
appropriately to what the header for that column says (i.e. if the column header states initials and date, then each box should have
the employee’s initials and the date the skill was completed). The first page of each section should have the employee’s name and
other requested information on that page written legibly.
There is a column labeled as “initial self-assessment validation”- this is to be completed by the orientee before starting in the work
area. This should be done during the first week of orientation as a way to tailor the orientation experience. The next column is labeled
“performance validation”. This section will be completed with the preceptor to verify that the orientee has been exposed to the skill by
one of the validation methods listed. The next column is labeled “end of orientation self-assessment validation.” This will be completed
by the orientee once orientation is finished. The goal of this second self-assessment is to be able to mark “can do” for all the skills. If
the orientee marks “needs review,” it should be reassessed as soon as possible. The last column, “comments,” should be utilized for
any section that needs review. If the orientee does not feel comfortable with a skill or has not had “hands on” practice of the skill, the
orientee must bring in help from another experienced employee.
The completed Skills Validation Checklist should be turned into the management team of the department. It will be kept with the
employee’s competency file. This packet may be audited by accrediting agencies to deem competency in certain skills.
