1.A.2.a-c PERSON CENTERED PHILOSOPHY
POLICY:
It is the policy of the Light of Truth Center to embrace a person centered philosophy. This philosophy governs the use of a person-centered, love-based approach. LTC’s philosophy embeds a person-centered approach in all policies, procedures and support services. LTC’s person-centered approach ensures participants and staff alike are at the center of planning, decision making and guides service delivery, as demonstrated by LTC ‘s mission, core beliefs and core values.
PROCEDURES:
1. The leadership and staff of the Light of Truth Center shall identify needs, aspirations, strengths of participants and develop plans with each person to achieve what is important. Support will be provided to ensure services are responsive and personalized.
2. The quality assurance team of the Light of Truth Center, Inc. shall continuously observe and monitor services provided. A person-centered approach is supported through continuous review, improvement and professional development.
3. LTC’s staff provides opportunities for participants to provide relevant feedback, and suggestions presented by the participant counsel.
4. The participant counsel’s representative attends monthly board meetings and reports participant concerns and suggestions.
5. Participants and staff will be at the center of all decision making and planning that assists them to achieve a healthy, sustainable lifestyle.
6. Family, friends, significant others and other service providers, who have a commitment to a better life for the participant can be invited to be actively involved in the development and implementation of the person’s plan per the participant.
7. The Light of Truth Center, Inc. will provide ongoing support to staff to ensure consistent quality outcomes are achieved and person centered service delivery is communicated in a clear, understandable manner.
8. Person-centered services start with the leadership of an organization, is sustained by middle management and is maintained by front line service providers. Person-centered services support personnel and participants alike valuing individual needs and contributions.
