3.6. Reimbursement of medical

3.7. Hospital confinement
expenses - P 500.00/day
Section 4. Procedures in claiming benefits
4.1. Submit to the Office of the Student Affairs and Services
(OSAS) a written report of the accident and same shall be
forwarded to the Accounting Office for the preparation of
disbursement vouchers.
4.2. Submit required documents such as the following:
4.2.1. In case of death due to natural causes:
4.2.1.1. Death Certificate
4.2.1.2. Certificate of enrollment signed by the
Registrar
4.2.2. In case of accident (not related to school activities):
4.2.2.1. Police report/Barangay report
4.2.2.2. Medical Certificate & Hospital Bills
4.2.2.3. Certificate of Enrolment signed by the
Registrar
