management of your practice after your death, pending its disposal or
cessation? [Note: The manager must hold an unconditional practising
certificate and must have consented. The manager has to counter sign the
prescribed form.]
• Have you notified the Society (within 14 days of commencing practice) of
the name, address, telephone number, fax number, telex number and DX
number of the executor of the will containing the testamentary provision
and of the solicitor? If the will has been lodged with another person, the
name and address of that person?

19. LAND REGISTRY (if applicable)
If required, contact The Land Registry for allocation of a Solicitors Code
number for the firm.
The Solicitors Code must be filled in the E-memorial Form (or the pre-
printed memorial form that can be purchased from the Land Registry) when
the firm lodges an instrument with the Land Registry for registration.
If required, read the Computerized Memorial Day Book.
20. COMPANIES REGISTRY (if applicable)
If required, open an account with the Companies Registry for the purpose
of electronic filing using the e-Registry.
